All forms under the Athlete Participation Forms TAB should be submitted online and are required to participate in athletics. The physical (MA School Heath Record) is also required and can be found under the Download and Print TAB.

Filling out online forms

• To access the online forms click on the “Athlete Participation Forms” TAB. You will see a list of required forms to fill out or read and consent. Please read each form carefully.

• To sign the document click inside the signature box and hold your mouse down, this will allow you to create an “Electronic Signature”. If you make a mistake and need to start over, click on the refresh icon next to the signature box.

• Once you have filled out each form and signed at the bottom, click submit. You will have the opportunity to print that page.

• Enter your email address in the box provided and you will also receive a confirmation email.
